No. Haven't seen anything like that.

If you "can't get rid of the window" it sounds like your computer got infected with something.

Check your browser's add-ons to make sure something you don't want didn't install in there (like Yontoo or something).

Try running Malware Bytes or some other Adware detecting software if you don't see a toolbar.

I ran Malware Bytes but it's still happening. How do you check the browser add-ons?

I'm going to try Add Blok Plus which was suggested in a post I just read and see if that will help.

What do you use as a browser?
Firefox? IE? Chrome? Opera?

Check here
https://www.mozilla.org/en-US/plugincheck/

or here
https://www.mozilla.org/en-US/plugincheck/

You need to check both "extensions" and "plug-ins" in the Firefox Add-Ons Manager.
The Add-ons Manager looks like this

I have nplbinst (v 1.0.0) and Rocket Life Secure plugin Layer. I didn't see a Firefox Add-)n Manager though.

The Firefox "Add-Ons Manager" opens in a separate tab that is labelled "Add-ons Manager." It opens when you select "Add-ons" in the Firefox menu. It's not a separate program. You may have already found it but not realized what it was called.

Maybe this video will help you find it / identify it.

Rocket Life Secure and nprlsecurepluginlayer.dll seems to be softare for digital photos.
http://www.shouldiblockit.com/nprlsecurepluginlayer.dll-4430.aspx

So I don't think either of them are causing the problem.
However you can try temporarily disabling them and see if that helps eliminate the "surveys."

You may also want to delete browser cache, in case there's something in there that is causing the pop-ups. However deleting browser cache may delete cookies that allow you to automatically log into websites.
Here are instructions:
https://support.mozilla.org/en-US/kb/how-clear-firefox-cache
or
http://pcsupport.about.com/od/maintenance/ht/clearcacheff.htm

Check here if you haven't already
http://malwaretips.com/blogs/remove-survey-2015-virus/
It gives the names of some of the malware that appears in the Programs menu.

Looks like I'm finally rid of the survey pop up. I did a bunch of things so I'm not really which one worked, but what a relief.

Thank you, draclvr and Jenny100 for all your help, it was very much appreciated.
