Have you talked to the boilerman completely?

You have to learn a lot of things from him and Samuel has to say he wants to see James' cell.

Have you tried going back and talking to the nurse and boilerman again? Sometimes you have to talk to people several times before they say everything. Also, have you checked out both parts of the graveyard behind the Sanitarium...

Just to add - try to enter the grilled gate to the left of the nurse's station and see that you can't go through. That might give Samuel ideas to try another ruse.
