The blue plastic bag is on the pavement in front of Oleg's house, just to the right of the yellow phone box and to the left of the two filled blue bags.

ive tried to get the guard to talk about smoking to give me a cigarette but he does not mention it, how can i get the cigarette

Have you both left and right clicked on the cigarette butts on the ground? Also have you examined his newspaper, and picked up the pebble closeby?

ive done the newspaper and got pebble, but forgot butts on floor, i will try that

when i go to gate, the nurse wont let me in

Are you in Cuba now? You won't reach the inner courtyard from inside the main building. You need to exit outside, and then head for the far right area, I think. As the walkthrough says: Walk through the gate with the Ernesto Che Guervara portrait.
