Ewil posted a solution over in Discussions a short while back, see if it helps:

For those of you who are having problems, when you download the file, it is downloaded as Blue_Madonna_demo without any extension. When you try to open or run it, it asks you what program you want to use to open it. Just right click on the file and hit 'rename' and add .exe to it. You should then be able to run it when you double click it. When I downloaded the file, I used a download manager, and it opened and ran okay without any issues.

Have you downloaded all the links stated in the mail sent to you?
Place all the files downloaded in a folder.
Then if all are downloaded already and in one folder - click on the file #1.
That file should open the set up - bin files.

I downloaded it without any trouble, but why can't it be downloaded like BF does to install the game.
I didn't download the free download manager. Just made a new folder in my local disk (c) and clicked on each file and put it into the new folder I made. name it whatever you want. Much easier. click on setup.exe. to install the game. Running win xp.

There seems to be some mix ups between the full game and the demo here. In post 2, Menahune is talking about the full game, while Rushes is quoting a discussion about the demo from another post.

With the demo, some users ended up with a file without an extension name (i.e. "exe") in their install folder. I have no explanation for this, other than it probably was some quirk at the server where it was stored. I've tried to fix the problem by uploading to a new server.

Obviously, some players have problems installing the on line version of the game from The Adventure Shop. I'm not an Adventure Shop costumer, and don't know how they've chosen to distribute the game files. But if someone has problems after trying the methods described in MaG's and Kjos' posts above this, feel free to contact me, and I guarantee that we can solve it somehow.
